The Italians got on the board first thanks to a goal from the Spanish Pedro, but Pavlidis and Kerkez turned the tide.
In the rematch, which will be played on Thursday, November 16, in Alkmaar, the Italians will need to mount a comeback.
Early in the first half, Lazio seized the lead. Pedro Rodriguez of Spain volleyed a cross from Mattia Zaaccagni through the penalty area and into the centre of the goal.
AZ tied the game at the end of the first half. Pavlidis was hit to even the marker while unmarked.
Milos Kerkez was able to finish with a low shot in the centre of the goal after receiving a wonderful ball from Jesper Karlsson, giving the Dutch the victory.
